I noticed you said Paste Insert and Paste Write are the same. The are close but not the same. The difference is that in non-resizable streams (such as in disks) past[e] write overwrites while paste insert will not work, or actually revert back to behaving like paste write. Maybe I should just disable paste insert when actual inserting makes no sense, instead of making it an alias for paste write. What do you think?"
YES, for editing disk drives (or streams) where "Paste Insert" makes no sense, it would definitely be a good idea to "gray out" (disable) that option, and have only "Paste Write" be available for writing!
