I've manually added a context menu to my Win10Hx64 which enables opening a file in HxD.
I've U/L'd a screenshot for same here:
I've attached a .reg file with the needed additions to the registry.
While the attached is pretty benign, <insert normal disclaimers here> - use at your own risk, as I am not liable for your borking your OS...
I offer it, in case anyone else might be interested in having this, but hasn't yet implemented it themselves, and is unable to get it to work via the app's provisions.

I have the same problem that I cannot get the Tools -> Options General context-menu settings to take effect, even if I run HxD as administrator. It used to work, but is not working with the latest 2.3.0.0 (x86-64) just downloaded with Microsoft Windows 10 Pro 64-bit [Version 10.0.18362.356].
The registry edits provided do work.
